Important Information
  1. Only Owner should be applicant and certificate will be issued on his/her name.
  2. All the Photos and Signatures and Documents that will be uploaded should be in .jpg, .jpeg and .pdf format and maximum file size should be 200 KB.
  3. Fire safety and pollution certificate maximum file size should be 500 KB.
  4. For Identity proof, any one of these - Driving license/PAN Card/Passport/Ration Card/Aadhaar Card/Voter Card are required.
    5. Required Documents
    1. Landlord Agreement is required in case of private land.
    2. Photo,Signature,Identity proof and Legal Identity Proof of Owner.
    3. Photo,Signature,Registration Certificate,Consent Letter and Pass Certificate of IN-Charge.
    4. Photo,Signature,Registration Certificate,Consent Letter and Pass Certificate of all Doctors.
    5. Photo,Registration Certificate,Consent Letter and Pass Certificate of all Paramedics.
    6. Undertaking of "No past convictions" / pending criminal cases.
    7. Ownership Proof / Landlord Agreement(5 years).
    8. Document of Authorization from State Pollution Control Board.
    9. Fire Safety Certificate or Declaration.
    10. Undertaking for maintenance of records.
    11. Undertaking for providing free treatment to BPL patients.
    12. Undertaking for treatment of patients in emergency.
    13. Declaration of CGHS rate chart.
